Change Form Template

How, and why, to track changes at your scaling startup.

Employee changes happen quickly and often in early-stage startups: titles change, role responsibilities pivot, and teams get reorganized as the company grows. One way we recommend keeping track of the changes in your organization, as well as protecting yourself from future confusion or false claims, is to document changes in a simple, standardized format.

An Employee Change Form should be used to document why employment decisions were made, when they took effect, and to track the employee’s acknowledgement of the change. Some examples of when change forms are best utilized:

  • Promotions

  • Title changes

  • Management changes / changes in reporting structure

  • Department changes

  • Compensation changes

  • Employee complaints & investigations

  • Long term leave requests (sabbatical)

*Note that you might also track the same information for things like terminations and performance improvement or corrective actions but these will likely be kept in different document formats with more extensive information.
